SUBMIT ALL SPEAKER INFORMATION BELOW 

We will need the following information provided by you: 

1. Speaker Information form, such as credentials, headshot and bio. An email will be sent out to you with this request. Due Monday, May 1st, 2024 

1. Faculty Disclosure Form Due Monday, May 1st, 2024 

1. COPE Activity Form (we need 1 form for each presentation you are providing) Due Monday, May 1st, 2024 

1. Submit a copy of your presentation and handouts Due August 31st, 2024

PRESENTATIONS 

All powerpoint presentations are due Friday, August 31st, 2024 by uploading the files below in the speaker submission form. You can upload multiple files to the box below.

COMPED REGISTRATION & LODGING  

All Vision by Design speakers receive comped registration – this registration is already taken care of for you and you do not need to register. 

AAOMC will make your hotel reservation for you, and cover the nights per your agreement. All additional nights are at your own expense. Please provide us with your dates of stay in your speaker information form. If you have any changes, please contact megan@aaomc.org.  

PRESENTATION DAY REQUIREMENTS 

• Please arrive 20 minutes before your talk and check in at the registration desk 
• A PC laptop and slide advancer will be provided for you 
• Please bring a backup copy of your presentation on a USB drive if possible 
• If you made any changes to your presentation since submission, please also bring a copy on a USB drive 
• If you have any special sound requirements please let us know
